You should have the best care possible—in the hospital and out. To help bring home health and hospice services to more patients and families, Wentworth-Douglass Hospital partnered with Amedisys to offer comprehensive at-home nursing, therapy and hospice services through Wentworth Homecare and Hospice. In 2013, Wentworth Homecare and Hospice was recognized as one of the top 100 Care Centers in the nation by HomeCare Elite.

How Does Home Health Care Work?

• Once you choose Wentworth Home Care with your doctor's referral, a home health care transitions coordinator will visit you in the hospital to arrange services.
• Before you leave the hospital, or within approximately 24-48 hours after you arrive home, a nurse or therapist will contact you to schedule a time to come to your home.
• During the first visit, a Wentworth Home Care nurse will go over any instructions your doctor provided and see how you are doing. Before the visit, gather all your medications including any over-the-counter drugs or home remedies you may be using so the nurse can see them. It’s also helpful to have a family member or someone who knows your medical history present during the first visit.
• If you need services from a physical therapist, occupational therapist, speech therapist, social worker or a home health aide, the Wentworth Homecare nurse will arrange for these services after the initial visit. The number of visits made by Wentworth Homecare's staff is based on your physician’s orders and your medical needs.

Home health costs are generally covered by Medicare and many private insurance plans when you meet certain eligibility requirements. You have options for home health or hospice providers and may ask your doctor to refer you to any provider you choose.

What is Hospice?

Hospice is a compassionate form of health care that provides non-curative treatment and comfort to people with a life expectancy of six months or less, according to a physician’s judgment. It is centered on caring for the whole person – with expert medical care, pain management as well as emotional and spiritual support for you – along with resources, information and emotional support for your family.

This compassionate care is about providing comfort, dignity, and quality of life. Wentworth Hospice goes to great lengths to help patients and their loved ones live each day to its fullest. The Wentworth Hospice team understands that as you approach the end of life, every moment matters.

How Does Hospice Work?

This expert care is provided in the comfort of your own home – wherever you call home. The Wentworth Hospice team of physicians, nurses, hospice aides, social workers, chaplains, bereavement counselors and volunteers work together to meet your unique needs. They meet you where you are – seeking to learn what matters most to you at this stage of your illness – so they can deliver the best possible care for you.
